Behavioral and Medical Healthcare: Community Gaps

• What is the mission of your company or project?: Our mission is to promote the mental and physical wellbeing of the individuals and families we serve, and ultimately that of our community. We believe that if we serve our community with diligence, love and respect, our community will grow along with us.
• Describe the problem you are solving:  Much of San Bernardino county is classified as a vulnerable and underserved population, according to the Department of Health and Human Services (HHS). People of San Bernardino county lack access to health care. Even when available, care is unaffordable and not provided in a timely manner.
• What is the market size/opportunity? 

  Our clinic in located in the city of San Bernardino, California and open to serving the over 39,512,223 people in California and the 2,180,085 people in San Bernardino County. 

  According to the 2020 community health status data report, 92.3% of residents in California and 90.9% of residents in San Bernardino County have health insurance. However, 14.4% of California residents and 14.7% of San Bernardino county residents experience delay in receiving healthcare. The report also states that, in San Bernardino County, there are 1,747 people for each primary care physician. Our mission is to bridge that gap and ensure that everyone who needs healthcare is able to get access to the services they need in a timely manner.

  We understand that, as of 2019, 35.7% of San Bernardino residents speak Spanish at home, 5% speak Asian and Pacific Islander languages, 1.5% speak other Indo-European languages, and 0.9% speak other languages. That is why our state-of-the-art software and language service operators are always available to assist with your language needs at each stage of your visit.

  There were approximately 23,000 low-income residents in need of mental health services in the years 2017 and 2018, who did not get care. Currently more than 1 in 3 adults in the United States lives with cardiovascular disease, which includes arterial disease, high blood pressure, cardiac arrest and arrhythmia. The percentage of San Bernardino County residents diagnosed with cardiovascular disease increased from 6.9 percent in 2015 to 7.6 percent in 2019, 13.9% have diabetes, 12.9% have asthma and 34.1 have obesity. Our integrated healthcare system is specifically designed to diligently manage these problems and coordinate your care, such that your needs are all addressed in 1 visit or effortlessly coordinated.
• What is your company/product about and how does it solve the problem? 

  Our integrated healthcare model allows us to manage patients with behavioral, counseling, and medical problems; offering both in-person and telehealth services. This would allow community members access to behavioral, counseling, and medical care in a timely, and diligent manner under one roof.

  Our psychiatric services include management of patients with psychiatric disorders across the lifespan. Common diagnosis include depression, anxiety, bipolar, and ADHD.

  Our medical services include management of patients with non-urgent (primary care) and urgent medical problems. Our nurse practitioners are experienced in treating diabetes, high blood pressure, thyroid diseases, cholesterol, urinary tract infection, asthma, cough and colds, lacerations.

• We shall be providing counseling services through our partnership with Youth Hope Inc. Counseling services shall include marriage and family therapy, drug/alcohol education and treatment, smoking cessation, domestic violence, individual and group counseling, parenting classes, case management, and behavior management.

• Why are you seeking funding and how will you spend the funding? The primary focus of Diligence Behavioral and Medical Integrated Healthcare is to increase timely access and affordable medical and mental healthcare for children, adolescents, and adults in the underserved communities of San Bernardino and its environs. The money that you would generously provide shall go towards achieving this goal. We shall be spending some of the money in community outreach programs, advertising, and marketing to ensure that community members are aware of our existences and the services that we provide.  Some of the money would go towards paying the lease for our office space, so our members could have a place to go when they require a face-to-face visit with their provider. We shall also spend some of the money in buying office equipment and furniture, such as exam beds, waiting room chairs, educational electronic monitor for the waiting room, papers, copy machine etc. The rest of the money would go towards procuring an electronic health record (EHR) software which we would use to collect and secure our members’ health records, and easily coordinate their care with other providers and specialists.

Who is on your team? 

Our team is made up of three women and two men. The three women are Dr. Oluwatoyin Ogunjale, Mavis Efuetngu, and Bernadette Akpengbe. The two men are Dr. Tony Akpengbe and Dr. Eric Efuetngu. All our providers are board-certified nurse practitioners and have licenses as Psychiatric Mental Health Nurse Practitioners (PMHNP), Adult-gerontology Nurse Practitioners (AGNP), and Family Nurse Practitioners (FNP). Together we have over 14 years of advanced nursing practice experience and have worked in ER, inpatient and outpatient psychiatric, long-term care, primary care, weight loss clinics, acute care hospital, telehealth, etc.
